Elkhart County Health Department Child Immunization Brochure

The Elkhart County Health Department (ECHD) immunization clinic offers vaccines based on the recommendations from the Advisory Committee on Immunization Practices (ACIP). For more information on vaccines and the diseases they prevent go to the Centers for Disease Control and Prevention (CDC). For information on Indiana school immunization requirements go to Indiana Department of Health (IDOH).

Effective March 2022, ECHD can see children who have private insurance as well as those who qualify for the Vaccines for Children (VFC) program. VFC is a federally funded program that provides vaccines to children who might not otherwise be vaccinated because of inability to pay.

Eligibility, Fees, and Insurance

  • There is a $15.00 administration fee per vaccination, subject to change without notice. Clients who qualify for the VFC program will not be denied services due to inability to pay the administration fee. These fees will be billed to insurance or Medicaid for those with coverage.
  • Payments are can be made by check, cash, or credit card.
    • Credit card payments will be charged a transaction fee.
  • Clients under 19 years are eligible to receive vaccines from ECHD if they meet one of the following:
    • Eligible for Medicaid
    • No health insurance
    • Underinsured: insurance does not cover vaccines
      • Verification from insurance company is required
      • A high deductible plan is not considered underinsured
    • American Indian or Alaska Native
    • Private insurance

Important Requirements to be Seen:

  • Children under 18 years of age must be accompanied by a parent/guardian or an adult 18 years or older with written permission from a parent.
  • Complete shot record showing all immunizations received since birth needs to accompany child.
  • Photo ID
  • Bring insurance/Medicaid cards to the appointment.
  • If you are 10 minutes late for an appointment you may be asked to reschedule.

  • Clinton Township Fire Department, Millersburg, IN
  • Clinton Frame Church, Goshen, IN
  • First Mennonite Church, Middlebury, IN
  • Nappanee First Brethren Church, Nappanee, IN

Immunization Records

All requests for immunization records will be processed through the Elkhart Office. There will be a $5.00 charge, per client, for a certified copy of the immunization record. Additional copies, per client, requested at the same time will be $3.00 each. Fees subject to change at any time without notice.
