Keeping Children Safe with Immunizations

Vaccines are given based on guidelines from the Indiana State Department of Health (ISDH) and the Centers for Disease Control and Prevention (CDC).

The Vaccines For Children (VFC) program is a federally funded program that provides vaccines to children who might not otherwise be vaccinated because of inability to pay.

Refer to the HPV: Diseases and the Vaccines that Prevent Them handout for more information on the HPV (Human Papillomavirus) vaccination.

NOTE: Children under 19 years are eligible to receive vaccines through the VFC program if they meet one of the following criteria:

  • Eligible for Medicaid
  • Uninsured
  • Underinsured
  • American Indian or Alaska Native

Your child CANNOT get VFC vaccine if your insurance covers the vaccine EVEN IF:

  • You have a high deductible
  • You have a co-payment

IMPORTANT:

A child must be accompanied by a parent/guardian or an adult 18 years or older with written permission from a parent.

  • Complete shot record showing all immunizations received since birth needs to accompany child.
  • Must meet vaccine eligibility guidelines.
  • If you are 10 minutes late for an appointment you may be asked to reschedule.
  • Payments are made by check, cash, or credit card.
  • $15.00 per vaccination, subject to change without notice.

  • Clinton Township Fire Department, Millersburg, IN
  • Clinton Frame Church, Goshen, IN
  • First Mennonite Church, Middlebury, IN
  • Nappanee First Brethren Church, Nappanee, IN

Immunization Records

All requests for immunization records will be processed through the Elkhart Office. There will be a $5.00 charge, per client, for a certified copy of the immunization record. Additional copies, per client, requested at the same time will be $3.00 each. Fees subject to change at any time without notice.
