Vaccines are given based on guidelines from the Indiana State Department of Health (ISDH) and the Centers for Disease Control and Prevention (CDC).
The Vaccines For Children (VFC) program is a federally funded program that provides vaccines to children who might not otherwise be vaccinated because of inability to pay.

NOTE: Children under 19 years are eligible to receive vaccines through the VFC program if they meet one of the following criteria:
Your child CANNOT get VFC vaccine if your insurance covers the vaccine EVEN IF:
A child must be accompanied by a parent/guardian or an adult 18 years or older with written permission from a parent.

All requests for immunization records will be processed through the Elkhart Office. There will be a $5.00 charge, per client, for a certified copy of the immunization record. Additional copies, per client, requested at the same time will be $3.00 each. Fees subject to change at any time without notice.
