The mission of the Elkhart County Vital Records office is to provide friendly, courteous, and accurate service in issuing birth and death records. This office holds records from 1882 to the present for births and deaths that have occurred only in Elkhart County. The Indiana State Department of Health-Vital Records maintains records of persons born in the State of Indiana from October 1, 1907, to the present.

Attention: The Elkhart County Health Department utilizes Permitium LLC to manage online transactions for purchasing Birth and Death Certificates. Permitium LLC is authorized by the state of Indiana to provide point of sale services to local health department vital records offices. Elkhart County Health Department will not process any online applications using other unapproved vendors.

No, apostilles are processed with the Secretary of State’s office. You can find those on their website.
The Elkhart County- Vital Records only maintains records for persons born in Elkhart County. Please contact the county Vital Records in the state where you were born. Your state may use Permitium.
An Affidavit Requesting Amendment can be done to amend any minor changes to an individual’s record or their minor child’s record(s). Two documents (over 10 years old) must be presented to confirm the spelling of the name. There is a $20.00 amendment fee plus the cost of a new birth certificate. Please come in prior to 3:00 pm weekdays to allow for adequate time to do an Affidavit Requesting Amendment. Please feel free to call our office with any further questions.
Even though a person was adopted in Elkhart County, the record is maintained in the county/state that the birth took place in.
Under these circumstances, a member of your immediate family may be eligible to apply for your birth certificate for you by presenting their identification. They must also show proof of the relationship (ex. birth certificate or a marriage certificate). See Identification Requirements. You can use Permitium or call our office for more information
Death records are recorded in the county/state where the death occurred, no matter where he/she lived or is buried.
For your convenience, you can order and pay online through Permitium. An additional fee is charged by Permitium for this service and all major credit cards are accepted.
PERSONAL CHECKS ARE NOT ACCEPTED. Payment must be in cash, certified check, or money order payable to: Elkhart County Treasurer. Visa, MasterCard, American Express, or Discover are accepted in the office (no telephone or fax orders). There is a convenience fee when using a credit/debit card.
