The Elkhart County Health Department (ECHD) immunization clinic offers vaccines based on the recommendations from the Advisory Committee on Immunization Practices (ACIP). For more information on vaccines and the diseases they prevent go to the Centers for Disease Control and Prevention (CDC). For information on Indiana school immunization requirements go to Indiana Department of Health (IDOH).
Effective March 2022, ECHD can see children who have private insurance as well as those who qualify for the Vaccines for Children (VFC) program. VFC is a federally funded program that provides vaccines to children who might not otherwise be vaccinated because of inability to pay.
Vaccines are provided by appointment at the following locations:
All requests for immunization records will be processed through the Elkhart Office. There will be a $5.00 charge, per client, for a certified copy of the immunization record. Additional copies, per client, requested at the same time will be $3.00 each. Fees subject to change at any time without notice.
Immunization Records Request Forms and a link to the Indiana Immunization Information Systems (My Vax Indiana) are located in the Important Links and Documents section of this page.
